Modifications to the subject Attachment are as follows: ➢ Transfer of Fields NC-RD-03-01 through 05 in Randolph County from Permit No. WQ0004825,to be included in this permit as NC-RD-43-01 through 03. ➢ Transfer of Fields NC-RD-46-01 and 02 in Randolph County from Permit No. WQ0004825. The modified Attachment B shall be effective from the date of certification until September 30, 2025 (i.e., expiration date of the current permit), shall replace Attachment B of Permit No. WQ0001684 issued October 10,2019,and shall be subject to the conditions and limitations as specified therein, as well as those specified in the above-referenced permit. Please replace Attachment B of Permit No.WQ0001684 that was issued October 10,2019 with the modified Attachment B. Please note that the Permittee shall submit a renewal application no later than April 3,2025. Please note that only the maps for new land application sites are attached to this letter. It is the Permittee's responsibility to ensure that a complete permit is created by inserting these documents into the previously issued permit. If any parts, requirements, or limitations contained in this certification are unacceptable, the Permittee has the right to request an adjudicatory hearing upon written request within 30 days following receipt of this permit. This request shall be in the form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes, and filed with the Office of Administrative Hearings at 6714 Mail Service Center,Raleigh,NC 27699-6714.
